"WORK AUTHORIZATION"
Aircraft Acknowledgement Statement
I hereby authorize the service and repair work to be performed, along
with the necessary parts and materials to be used, as well as any sublet work
necessary, to determine the cause and correct any discrepancies either
specified by customer or discovered by The Ohio State University Airport
(OSUA). I hereby grant OSUA permission
to operate, tow, or fly aircraft for the purpose of testing and
inspection.
If aircraft is returned to customer prior to the completion of repairs
or service, a diagnostic charge and handling fee, along with charges for
completed labor and parts used, will be collected. I understand that exchanged part prices are
based on an acceptable core and agree to reimburse OSUA at a later time if my
core is rejected. I understand that
discussed charges are only an estimate (approximate calculation) and the actual
charges may exceed the estimate. A token
charge of 5% of the labor charges may be collected for shop supplies used such
as nuts, bolts, cotter pins, rags, grease, solvents, etc. Actual shipping will be charged.
The limited warranties applying to the parts listed hereon are those
that may be offered by the manufacturers.
OSUA hereby expressly disclaims all warranties, either expressed or
implied, including any implied warranties of merchantability or fitness for a
particular purpose and assumes no liability in connection with the sale of
these parts or services. Customer shall
not be entitled to recover from OSUA any damages to property, damages for loss
of use, loss of time, loss of income, or any other incidental damages.
All charges are due and payable simultaneously with the delivery of
aircraft, or within 3 days of notice of completion of service. Title to materials used shall not pass to
customer until payment in full is received by OSUA. An express mechanic’s lien is hereby
acknowledged on aircraft to secure the amount of parts, repairs, service and
labor.
OSUA will not be responsible for damage or loss of aircraft, or contents
left in aircraft, in case of fire or theft, or any cause beyond their
control. If it becomes necessary for
OSUA to employ an attorney to collect these or any charges, I agree to pay all
costs and fees, including court and attorney fees associated with your actions.
If customer leaves keys with OSUA for ease of future repairs and
service, then this agreement shall be continuing in nature and shall remain in
force until cancelled in writing. If I
pay by credit card, I authorize charges to the card described above and agree
to perform the obligations in my cardholder agreement with the issuer. The laws of the state of
